When consult for the first time
When we never had sexual relations
Synthesis of a discussion in November 2002 about Gynelist between Dr.Michel Briex (Libourne), Bernard Cristalli (Evry, France), Thierry Dumont (Hyeres), Christian Fossat (St Martin), Charles Nahmanovici (Nice), Stephane Oks (Montpellier) Luc Pacaud, Gérard Samama (Rueil), Joel Silvert (Valenciennes), Bob Tetuanui, Bernard Tiberghien (Grasse).
In the absence of gynecological problem, no consultation at the gynecologist is necessary.
Consult only
- In case of problems:
Painful menstruation
Pelvic pain
Menstrual irregularity few years after puberty
Amenorrhea after 15 or 16 years
- Or when the first sex is envisaged
But we can advise mothers to bring their daughters with them at the gynecologist to 13-15 years depending on their maturity simply for a teaser, so that these young people (future) women see that the gynecologist is easy affordable and they know where to go if necessary in a more “urgent”.
It is explained that the gynecologist is bound by professional secrecy and it is there to help them if needed.
How does the consultation
The first contact with the gynecologist is very important. It’s an opportunity to talk, to put into trust.
It insists on effective contraception and prevention of STDs.
The presence of the mother (or the big sister, aunt, etc.) is desirable at the beginning of the Cs. But it is often necessary to reserve a period of intimacy allowing the girl to speak freely.
In the absence of symptoms, examination of a young virgin woman is useless unless the request because it needs to be reassured, for example. The totality of the consultation can be devoted to discussion.
If symptoms, or if the disease is suspected, and that a review is needed, we do not practice vaginal touching. The Touch Rectal is possible and is well accepted as it is explained in advance, and with gentleness and tact necessary but ultrasound through sus-pubienne is usually sufficient.
If leucorrhées or bleeding, it is possible to use a speculum (speculum of virgin) to explore the cervix and vagina.
The cervical smear is not necessary before 6 months of sexual activity.
